The clinic is now located at 92 Queen Street, Waiuku (sharing premises with Aurora Law). We have onsite parking to make it easier for our patients. So please drive in the driveway and park in any of the parks at the front of the building. Then you will find the clinic up the green astro turf covered wooden ramp on the left-hand side.
New Patients – prior to your appointment you will be sent (either via email or SMS) two secure links to both a New Patient Form & a Medical Questionnaire to complete. Please complete these & submit back to us prior to your appointment. There is no need to print off the forms.
Payment in full is required on completion of your appointment. We understand things crop up, but if you are unable to pay on the day, please contact us prior to your appointment to discuss.
If you don’t see an appointment that suits, please phone us at the clinic on 092352060. As we may have appointments that have become available that will suit you.
Cancelled & or rescheduled appointments – We understand that things do sometimes come up & appointments need to be cancelled or rescheduled. However we would appreciate you giving us as much notice as possible for any appointment changes.
DNA’s & late appointment changes/cancellations could be subject to a $55 DNA/Late cancellation fee.
Please note – If an incorrect appointment type is booked, we reserve the right to change it to the correct type.
